Medicaid enables states to provide medical and health-related services for individuals who meet income and other eligibility requirements. The Medicaid Program is designed to provide medical care for public assistance recipients and persons of low income who can meet their maintenance needs, but have insufficient income to provide the cost of medical care.
Individuals are eligible to receive Medicaid benefits if they meet non-financial requirements and resource and income limits for a specific covered group. Covered groups include the aged, blind, and disabled, and foster care and adoption assistance children. For more information about the Medicaid Program you may call the Suffolk Department of Social Services at 757-514-7450 or visit the Virginia Department of Social Services website at: www.dss.state.va.us then click Benefits and Medical Assistance.
You may apply for Medicaid at the Suffolk Department of Social Services, located at 135 Hall Avenue, Suite B in Suffolk, Virginia 23434, or request for an application to be mailed to you. An online application for Medicaid can be obtained by visiting the Virginia Department of Social Services website at: http://www.dss.virginia.gov/benefit/medical_assistance/forms.cgi
Uninsured children under the age of 19 years, who are ineligible for Medicaid, may be eligible for FAMIS.
