With limited exceptions Section 15.2-209 of the Code of Virginia states that persons seeking to present a claim for negligence against the City of Suffolk are required to provide written notice regarding the nature of the stated claim within six (6) months of the occurrence date.   In order for the City to properly investigate any claims a citizen may have, the City of Suffolk has developed the "Report of Incident & Claim Against the City" form for citizen convenience.  This form request information relating to:

• Claimant Name and Address
• Date and Time of Incident
• Individuals involved
• Location of Incident
• Why you believe the City is responsible
• Any proof of the claim, including bills, receipts, estimates, photos, statements, or any other similar information
